(d) Dr. Burns is a research scientist at
Hartford Hospital, CT. Previously, she was
a tenured associate professor of nursing and
director of the Center for Nursing Research
at the University of Connecticut, Storrs.
She has written or contributed to numerous
peer-reviewed publications on surgical
education, including the development of the
Advanced Trauma Operative Management®
(ATOM®)course, preparation for responses
to terrorism, and end-of-life care in trauma.
FRANK K. BUTLER, JR., MD, FAAO, FUHM
(e) Dr. Butler is Chair of the Department
of Defense (DoD) Committee on Tactical
Combat Casualty Care, Joint Trauma System,
Washington, DC. He also co-chairs the
Decompression Sickness and Arterial Gas
Embolism Treatment Committee for the
Undersea and Hyperbaric Medical Society.
As director of the SEAL Biomedical Research
Program for 15 years, he has led landmark
projects, including programs to promote
refractive surgery in the military, the Naval
Special Warfare decompression computer,
and Tactical Combat Casualty Care. He is a
retired Captain, Medical Corps., U.S. Navy.
RICHARD H. CARMONA, MD, MPH, FACS
(f) Dr. Carmona served as the 17th U.S.
Surgeon General (2002–2006). He is
vice-chairman of Canyon Ranch, chief
executive officer of Canyon Ranch Health
Division, and president of the Canyon
Ranch Institute, Tucson, AZ. He also is
distinguished professor, Mel and Enid
Zuckerman College of Public Health,
University of Arizona, Tucson.

WILLIAM P. FABBRI, MD, FACEP
(h) Dr. Fabbri is medical director of
operational medicine, Federal Bureau of
Investigation. He has served in both field- and
hospital-based capacities in industrial and
transportation accidents, natural disasters,
and law enforcement critical incidents.
